error occured when FormBreeze tried to process this form.
For the webmaster of this site:
There are two likely causes of this
problem:
1. There is a problem in the HTML code of
your form. Your HTML page must contain code that
looks like this:
<form
... >
<input type="hidden" value="12345-1"
name="formbreeze">
where 12345 is your
5 digit account number.
NOT your email address. Your account number
appears at the top-right of the screen when you log into your control
panel.
and -1 is the number of
the form in your control panel (1-3 for standard accounts or 1-10 for
professional accounts).
Please check your HTML code to make sure
that this field is correct. If not, correct the problem and
re-upload your form page.
2. You have not entered the location (URL) of this
form in the FormBreeze Control Panel. You must list each
form you want to process in the Control Panel and make sure that the
number of the form matches the number specified in the HTML code (-1 in the example above).
After making any changes in the control
panel, be sure to click the "Save All Changes" button. Your changes will not take affect
until you save them.
